You are invited to complete a Rating Tool to assess children's mental health systems entitled " Rating Tool for Implementation of the System of Care Approach. " The purpose is to assess your community's or region's progress in implementing the system of care approach for children, adolescents, and young adults with mental health challenges and their families. Results from the Rating Tool will provide guidance about areas needing attention and technical assistance to improve systems of care. The administration of this tool is being facilitated by the Georgetown University National Technical Assistance Center for Children's Mental Health. Participation is entirely voluntary. You may choose not to participate at all or to leave the survey at any time. Regardless of your decision, there will be no effect on your relationships with state or local agencies or with Georgetown University or any other consequences. The Rating Tool is designed to be filled out by multiple respondents per community/region to obtain an integrated perspective on your progress in implementing the system of care approach. You are being asked to take part because of your involvement and understanding of the system of care in your area. Your individual responses will be completely confidential. All data will be reported in aggregate. States and communities will receive reports of the results of the survey that contain average responses for each question. No one will be able to link your name and your responses. If you agree to participate, you will be asked to complete one electronic survey with questions related to several major system of care implementation areas. This online survey should take around 20 minutes to complete. You will not be required or forced to answer any question. You may answer "don't know" or "not applicable" to any question you choose not to answer. Below please indicate below whether you agree or disagree to participate in the survey. Data Security, Risks, and Benefits In order to keep information about you safe, the data collected will be kept private and confidential. The Georgetown University National Technical Assistance Center for Children's Mental Health is the owner of the data collected or uploaded into surveys. No one besides the Georgetown University team will have access to the data. SurveyMonkey offers SSL encryption for the survey link and survey pages during transmission.
